It says right on the polling card that you should bring it, but you can still vote without it (the implication is that may be a bit of a hassle, but I don't know). Of course, that doesn't help if you don't HAVE it, as you can't read the disclaimer :)
Yup! For reference, I voted without my polling card today (it is lost somewhere in the living room, possibly binned), and they asked me for my name and address. I'd suggest bringing proof of address just in case, but they may be fine without.
