In the catagory of 'entertaining forms': on the Avon and Somerset Police Lost Property Reciept, the rather indelicate phrasing 'Name and Address of loser: *my name and address*'.
Much hilarity ensued.

LOL. Poor Michelle. I always wonder if these things are
coincidence
or if people designing forms have an, occasionally sadistic, sense of humour.
I mean, the ambiguity alone, let alone the innuendo, in government forms is too artfully judged to be just coincidence :)
